1.0 Out of Competition Testing I agree that a Doping Control Officer (DCO) (duly authorised by UK Sport or other appropriate organisation) may notify me in person and without prior notice at any time that I am required to provide a sample and that sample collection procedures will begin immediately. Where I am given prior notice that I am to be tested, I agree that sample collection must take place within 24 hours of initial contact by the DCO. I agree that I must give the sample in the presence of the DCO and that I am subject to both UK Athletics’ (UKA) and the International Association of Athletics Federations’ (IAAF) Anti Doping Rules.
2.0 Change of Address / whereabouts
2.1
In order that a DCO can contact me without delay, I agree to inform UKA immediately if I change address. I will give full details of my new address and contact details to UKA a minimum of 5 days before it changes. I accept that this is my personal responsibility.
2.2
I am aware that I need to advise UKA of at least one location per day for at least 5 days of the week with at least one hour or more at each of these locations.
2.3
I will keep UKA informed and updated of any changes to the five periods of time as detailed in 2.2 above.
2.4
I accept that if I fail to comply with points 2.1, 2.2 and 2.3 above I can be disciplined by UKA pursuant to its Anti Doping Rules.
2.5
Notice given under this clause must be provided to UKA's Anti-Doping Department by phone, fax, email, letter or text message.
3.0 Retirement
I agree to notify UKA if I decide to retire from athletic competition. I understand that under UKA's Anti Doping Rules my name will remain on the Out of Competition Register for a maximum of 12 months after retirement.
If my name is taken off the Out of Competition Register and I decide to return to the sport, I understand that I will be subject to testing in line with UKA’s Anti Doping Rules and the IAAF Rules for a period of 12 months before I am eligible to participate in any international competition.
4.0 Athlete Declaration
I agree that I will abide by the provisions set out in parts 1.0, 2.0 & 3.0 of this form and the provisions of the Anti Doping Rules.
I am fully aware that it is my responsibility to update UKA with my whereabouts. I undertake to submit to both Out-of-Competition and In-Competition testing and that this is a condition of being considered to represent the United Kingdom in international athletic competitions and of taking part in athletic events in the United Kingdom and abroad. I understand that UKA may test me at any time and is not restricted to only testing during my test slots. I declare that I have read, understood and will comply with the Anti Doping Rules of both UKA and IAAF.
